Calibration, Certification, & Documentation Compliance Program, Enrollment Form TIC-MS, INC., a metrology laboratory with headquarters in St. Louis, Missouri agrees to administer a Calibration Compliance Program (CCP) covering calibration, certification, and documentation of all tools registered in the program and bearing a program reference number on behalf of the following customer: The purpose of this program is to provide a continuous care service for all tools registered in the program. Customer may use the program to comply with the calibration, certification and documentation procedures defined in various Government, Military, and Private Industry contracts. TIC-MS, INC. agrees to provide a Metrology Laboratory that is ISO-IEC-17025 accredited and operates in consonance with ANSI Z-540, Air Force Tech Order 33-1-14, QS-9000, ISO-9002, AS-9000. TIC-MS, INC. agrees to use procedures promulgated from time to time by recognized authorities including but not limited to DOD, ANSI, DCASR, QS-9000 STDS, ISO STDS, ASME, A.F. Tech Orders, and specific private industry sources. TIC-MS, INC. agrees that when authorities are in conflict or when one authority is more stringent than another that TIC-MS, INC. will follow the more stringent procedure so that customer will not fall into non-compliance due to overlaps in published procedures. TIC-MS, INC. agrees to keep customer informed of any published procedure changes that become known to TIC-MS, INC. by incorporating those changes into the procedures of the laboratory so that the customer will not fall into non-compliance due to published changes made known to TIC-MS, INC. by any of the above listed authorities. TIC-MS, INC. agrees to provide documentation as described by addenda and attached hereto; and that documentation will be of sufficient scope and detail as to satisfy the requirements of the above listed authorities; and further that if customer shall have a specialized contract requirement that is outside the scope of the guidelines published by the above listed authorities that TIC-MS, INC. will provide such additional information as is required to satisfy such contract requirements at a reasonable and nominal cost. TIC-MS, INC. agrees to perform all work at its normally published prices and to give customer 60 days notice of any change in those published prices prior to putting any increases into effect. Customer agrees that the information that is contained in the program and all procedures enumerated in the program are of a proprietary nature and will not be divulged to any party that is not a supplier or customer of the parties to this agreement. Further it is understood that the CCP is trade mark protected and all documentation remains the property of TIC-MS, INC.. All non-calibration documentation must be returned to TIC-MS, INC. at termination of the program. Customer agrees to provide TIC-MS, INC. with appropriate information when customer’s personnel perform any work on the tools registered in the program so as to maintain the integrity in the program. Customer agrees to notify TIC-MS, INC. when any tool registered in the program is removed from service so that the integrity of the program may be maintained and so that TIC-MS, INC. may purge its files and return same to customer for archiving. Except for customer’s personnel all work on the tools, while in the program, will be performed by TIC-MS, INC. or customer designee. At customer’s request TIC-MS, INC. will include tools not normally serviced by TIC-MS, INC. in the program and to source the calibration, certification and documentation of those tools on behalf of the customer so as to keep the program intact and to ensure the integrity of the program records. TIC-MS, INC. will add 10% to such charges as may be incurred on the customer’s behalf as a records and handling charge. Customer agrees to preserve all stickers and other types of identification affixed to tools in the program and to prevent any alteration of any information contained on any such identification device. Customer agrees to hold TIC-MS, INC. harmless should any tampering of records or seals occur due to any circumstance beyond the control of TIC-MS, INC. William Alexander Program Administrator
